Responsibilities
- Conduct comprehensive intake assessments for new clients to determine eligibility for government assistance programs.
- Develop and implement individualized case plans to help clients achieve self-sufficiency and stability.
- Monitor and review client progress, making necessary adjustments to case plans.
- Advocate for clients by connecting them with community resources, legal aid, and employment services.
- Document all client interactions, assessments, and case plan updates accurately in the departmental database.
- Maintain strict confidentiality in accordance with state and federal privacy laws.
Qualifications
- Bachelor's degree in Social Work, Public Administration, or a related field is preferred.
- Minimum of 2 years of experience in case management, social services, or a similar government role.
- Valid Driver's License and access to a reliable vehicle for site visits.
- Strong knowledge of Texas state government regulations and community resources.
- Excellent verbal and written communication skills.
- Ability to work independently and as part of a multidisciplinary team.
