Responsibilities
- Provide weekend client assessments and case management services for vulnerable populations
- Coordinate emergency interventions and crisis response during weekend hours
- Maintain accurate client records and documentation in state-mandated systems
- Collaborate with weekday staff to ensure continuity of client care
- Conduct weekend home visits and community outreach as needed
- Participate in weekend on-call rotations for urgent cases
- Report on weekend service outcomes and client progress
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field
- 2+ years of direct client service experience
- New York State Social Work License (LMSW) preferred
- Availability to work Saturdays and Sundays (7:00 AM - 7:00 PM)
- Proficiency with state-mandated case management software
- Bilingual English/Spanish proficiency required
- Valid New York State driver's license
- Ability to pass background check and fingerprinting
