Responsibilities
- Manage client caseloads through intake, assessment, and service coordination
- Maintain confidential digital records in county databases with 100% accuracy
- Connect clients with community resources (housing, healthcare, food security)
- Generate monthly progress reports using CRM systems
- Collaborate with social workers and healthcare providers on integrated care plans
- Participate in bi-weekly team meetings and quarterly training sessions
Qualifications
- Associate's degree in Social Work, Public Health, or related field (or equivalent experience)
- 1+ years case management or customer service experience
- Proficiency with Microsoft Office Suite and data entry systems
- Valid Minnesota driver's license with reliable transportation
- Ability to pass background check and county clearance
- Excellent communication skills in English (bilingual Spanish strongly preferred)
- Flexible schedule availability including occasional evenings/weekends
