Responsibilities
- Conduct home visits and family assessments to evaluate safety risks and service needs
- Develop and implement case plans with measurable goals for at-risk families
- Coordinate community resources including housing, healthcare, and educational support
- Maintain detailed case documentation and compliance records per state regulations
- Collaborate with law enforcement, schools, and healthcare providers for holistic interventions
- Provide crisis intervention and emergency response during critical situations
- Participate in multidisciplinary team meetings and case reviews
Qualifications
- Associate's degree in Social Work, Psychology, Criminal Justice, or related field
- Minimum 1 year experience in social services, child welfare, or case management
- Valid Texas driver's license and reliable transportation for home visits
- Proficiency in Microsoft Office Suite and case management software
- Strong communication skills with ability to navigate sensitive family dynamics
- Bilingual (English/Spanish) preferred
- Ability to work flexible hours including evenings and weekends as needed
- Pass criminal background check and child abuse registry screening
