Responsibilities
- Conduct comprehensive health assessments and provide direct patient care in community settings
- Develop and implement evidence-based public health interventions for vulnerable populations
- Analyze epidemiological data to identify emerging health trends and prioritize resources
- Coordinate immunization programs and outbreak response initiatives
- Deliver culturally competent health education to diverse communities
- Collaborate with interdisciplinary teams to address social determinants of health
- Maintain accurate patient records and comply with HIPAA regulations
Qualifications
- Active California RN license and Bachelor's degree in Nursing (BSN)
- Master's in Public Health (MPH) or equivalent certification preferred
- Minimum 3 years clinical nursing experience with community health focus
- Proficiency in electronic health records and public health data systems
- Bilingual proficiency in Spanish, Mandarin, or Cantonese highly desirable
- Valid California driver's license and reliable transportation
- Ability to work flexible hours including evenings/weekends for community outreach
