Responsibilities
- Conduct thorough intake assessments and evaluations for new clients to determine eligibility for government assistance programs.
- Develop, implement, and regularly review individualized case plans to ensure client goals are met and progress is documented.
- Collaborate with external agencies, law enforcement, healthcare providers, and non-profit organizations to coordinate comprehensive care.
- Maintain accurate and confidential electronic case files in compliance with all county policies and state regulations.
- Provide crisis intervention and conflict resolution support during high-stress situations.
- Attend regular staff meetings, training sessions, and community outreach events.
- Monitor compliance with program requirements and report any anomalies to supervisors immediately.
Qualifications
- Bachelor’s degree in Social Work, Criminal Justice, Public Administration, or a related field required.
- Valid Kansas Driver’s License and reliable transportation required for site visits.
- 1-3 years of experience in case management, social services, or public administration preferred.
- Strong understanding of state and federal government assistance guidelines and eligibility criteria.
- Excellent written and verbal communication skills with the ability to translate complex information clearly.
- Ability to maintain strict confidentiality and adhere to ethical standards of practice.
- Proficient in Microsoft Office Suite and case management software.
