Responsibilities
- Assess client needs and develop individualized service plans for at-risk populations
- Coordinate community resources and advocate for client access to healthcare, housing, and social services
- Document case notes, progress reports, and maintain confidential records per county regulations
- Respond to crisis situations and provide emergency intervention support
- Collaborate with law enforcement, healthcare providers, and community organizations
- Conduct home visits and field assessments across Sedgwick County
- Participate in continuous training and quality improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- Valid Kansas LSW (Licensed Social Worker) or LCSW (Licensed Clinical Social Worker) certification
- Minimum 2 years professional social services experience
- Proficiency in electronic case management systems
- Excellent crisis intervention and de-escalation skills
- Valid Kansas driver's license with reliable transportation
- Bilingual (English/Spanish) preferred
- Ability to work flexible hours including evenings and weekends
