Responsibilities
- Conduct community health assessments and data analysis to identify emerging health disparities
- Coordinate vaccine outreach events and health education workshops in underserved neighborhoods
- Collaborate with local healthcare providers to improve chronic disease prevention programs
- Prepare comprehensive reports on health trends and intervention outcomes for state compliance
- Develop culturally relevant health materials in English and Spanish
- Facilitate stakeholder meetings with community organizations and government agencies
- Support emergency response coordination during public health incidents
Qualifications
- Bachelor's degree in Public Health, Nursing, or related field (Master's preferred)
- Minimum 2 years experience in community health or public health program implementation
- Bilingual proficiency in English and Spanish (written and verbal)
- Certified in Public Health (CPH) or willingness to obtain within 6 months
- Proficiency in data visualization tools (Tableau, Power BI) and EHR systems
- Valid Texas driver's license and reliable transportation for field visits
- Experience with grant reporting and compliance documentation
