Responsibilities
- Conduct comprehensive health assessments and develop individualized care plans for underserved communities
- Lead disease prevention programs including immunization campaigns and health screenings
- Coordinate emergency preparedness responses for public health crises
- Delocate community health education workshops on chronic disease management
- Collect and analyze epidemiological data for program evaluation
- Collaborate with cross-agency partners to address social determinants of health
- Maintain confidential patient records and ensure HIPAA compliance
Qualifications
- Valid California Registered Nurse (RN) license
- Bachelor of Science in Nursing (BSN) or Master's in Public Health preferred
- Minimum 3 years clinical nursing experience in community/public health
- Spanish bilingual proficiency required
- Experience with electronic health records (EHR) systems
- Certification in Basic Life Support (BLS)
- Valid California driver's license with clean driving record
