Responsibilities
- Develop and implement community health education programs targeting underserved populations
- Coordinate vaccination campaigns and preventive health screenings across Chicago neighborhoods
- Collaborate with local healthcare providers and community organizations to expand outreach
- Collect and analyze health data to identify emerging community needs
- Manage public health grant reporting and compliance documentation
- Train community health workers on culturally competent outreach strategies
- Represent the department at public health forums and community events
Qualifications
- Bachelor's degree in Public Health, Social Work, or related field (Master's preferred)
- 2+ years experience in community health or public outreach
- Strong bilingual skills in English/Spanish or other relevant languages
- Experience with health data collection and program evaluation
- Valid Illinois driver's license with reliable transportation
- Ability to work flexible hours including evenings and weekends
- Certification in Health Education (CHES) or equivalent preferred
