Responsibilities
- Provide comprehensive case management for vulnerable populations including children, families, and adults
- Conduct thorough psychosocial assessments and develop individualized service plans
- Coordinate with community partners, healthcare providers, and legal stakeholders
- Document case notes and maintain compliance with county and state regulations
- Provide crisis intervention and trauma-informed care services
- Participate in multidisciplinary team meetings and program development
Qualifications
- Master's degree in Social Work or related field
- Valid California LCSW or LCSW-R license required
- 3+ years of direct social work experience in public health or human services
- Proficiency in electronic health records and case management systems
- Experience working with diverse populations and cultural competency
- Knowledge of California welfare and social service programs
- Valid California driver's license and reliable transportation
