Responsibilities
- Assess client needs and develop comprehensive case management plans
- Provide crisis intervention and therapeutic counseling services
- Collaborate with community partners to coordinate resources
- Maintain accurate documentation and comply with regulatory standards
- Lead outreach initiatives for underserved populations
- Train and mentor junior social work staff
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- Minimum 3 years of clinical social work experience
- Licensure as a Licensed Social Worker (LSW) in Minnesota
- Demonstrated cultural competency and anti-racism practice
- Experience with electronic health records (EHR) systems
- Valid Minnesota driver's license
