Responsibilities
- Assess Medicaid eligibility and coordinate healthcare services for vulnerable populations
- Develop and implement individualized care plans with clients and healthcare providers
- Conduct home visits and outreach to ensure access to critical services
- Maintain accurate case documentation and comply with state/federal regulations
- Collaborate with medical providers, social services, and community organizations
- Track outcomes and report on program effectiveness to leadership
- Respond to urgent client needs during crises and emergency situations
Qualifications
- Bachelor's degree in Social Work, Nursing, Public Health, or related field
- Minimum 2 years case management or healthcare coordination experience
- Valid Indiana driver's license and reliable transportation for home visits
- Proficiency with electronic health records and case management software
- Strong crisis intervention and conflict resolution skills
- Knowledge of Indiana Medicaid programs and healthcare systems
- Ability to manage complex caseloads with competing priorities
- Clear background check and drug screening required
