Responsibilities
- Coordinate comprehensive care plans for veterans across medical, mental health, and social services
- Conduct initial assessments and ongoing case reviews for veteran patients
- Collaborate with multidisciplinary teams to ensure continuity of care
- Process and track veteran benefits claims with 100% accuracy
- Provide crisis intervention and emergency resource referrals
- Maintain detailed electronic health records in compliance with federal regulations
- Develop community partnerships to expand veteran support services
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field (Master's preferred)
- Minimum 3 years case management experience in healthcare or social services
- VA or federal government experience strongly preferred
- Valid California driver's license with clean driving record
- Proficiency in VA electronic health record systems
- Ability to obtain and maintain federal security clearance
- Certification in Veteran Case Management (AVCM) or equivalent
- Spanish bilingual skills required for diverse veteran population
