Responsibilities
- Coordinate veterans' healthcare benefits and disability claims with 100% accuracy
- Provide case management support for veteran rehabilitation programs
- Collaborate with multidisciplinary teams to develop personalized care plans
- Conduct eligibility screenings for VA housing and education benefits
- Maintain confidential medical records and documentation in compliance with federal regulations
- Advocate for veterans' needs within community healthcare networks
- Train families on available VA resources and support systems
Qualifications
- Bachelor's degree in Healthcare Administration, Social Work, or related field
- 3+ years of experience in veteran services or healthcare coordination
- VA claims processing certification (preferred but not required)
- Proficiency with VA IT systems (Vista, Blue Button)
- Clearance for handling Protected Health Information (PHI)
- Strong knowledge of federal veteran benefits programs
- Excellent communication and conflict resolution skills
- Bilingual in English/Spanish (highly desirable)
