Responsibilities
- Coordinate veteran benefits enrollment and healthcare access programs
- Manage complex case files ensuring strict HIPAA and federal compliance
- Collaborate with multidisciplinary teams to develop personalized care plans
- Conduct eligibility assessments for VA compensation and pension programs
- Provide crisis intervention and mental health support referrals
- Utilize VA systems (VistA, CPRS) for documentation and reporting
- Train veterans and families on available VA resources and services
Qualifications
- Bachelor's degree in Healthcare Administration, Social Work, or related field
- Minimum 3 years experience in veteran services or federal healthcare
- VA certification in Benefits Delivery at Discharge (BDD) preferred
- Expertise in navigating VA healthcare systems and regulations
- Strong crisis intervention and conflict resolution skills
- Proficient in electronic health record systems (EHR)
- Ability to obtain and maintain a government security clearance
