Responsibilities
- Provide comprehensive case management for veterans navigating VA benefits and services
- Collaborate with interdisciplinary teams to develop personalized care plans
- Conduct eligibility assessments for federal healthcare and disability benefits
- Document all interactions using VA-approved electronic health record systems
- Represent the VA at community outreach events and veteran-focused initiatives
- Analyze veteran needs data to improve service delivery processes
- Maintain strict compliance with federal privacy regulations (HIPAA/VISN policies)
Qualifications
- Bachelor's degree in Social Work, Public Administration, or related field
- Minimum 3 years' experience in veteran services or federal case management
- Proficiency with VA computer systems (Vista, CDW, etc.)
- Valid Pennsylvania driver's license
- Ability to obtain and maintain federal background clearance
- Certification in Veterans Service preferred (e.g., CVSO)
- Demonstrated knowledge of Title 38 regulations
- Exceptional communication skills with diverse populations
