Responsibilities
- Provide comprehensive case management services for veterans and their families
- Coordinate healthcare benefits and disability compensation claims
- Develop and implement individualized care plans for veterans with complex needs
- Collaborate with medical providers and community partners for holistic support
- Utilize federal systems to maintain accurate veteran records and documentation
- Conduct outreach programs to connect veterans with available resources
- Monitor compliance with federal VA regulations and quality standards
Qualifications
- Bachelor's degree in Healthcare Administration, Social Work, or related field
- 3+ years of experience in veterans services or healthcare coordination
- Knowledge of VA benefits systems (e.g., VBMS, VBA) and federal regulations
- Certification in Case Management (CCM) or Veterans Services preferred
- Proficiency in electronic health records and data management systems
- Ability to obtain and maintain a federal security clearance
- Strong interpersonal skills for diverse veteran populations
- Experience with trauma-informed care approaches
