Responsibilities
- Manage veteran benefit applications and claims processing with 95% accuracy
- Coordinate care transitions between VA facilities and community partners
- Conduct eligibility screenings and resource navigation for veterans
- Develop and maintain comprehensive veteran care plans
- Collaborate with interdisciplinary healthcare teams
- Track and report key performance metrics for program improvement
- Provide crisis intervention and emotional support services
Qualifications
- Bachelor's degree in Social Work, Healthcare Administration, or related field
- Minimum 3 years experience in veteran services or healthcare coordination
- VA claims processing certification (preferred)
- Proficiency in VA electronic health records system (VistA)
- Strong crisis intervention and conflict resolution skills
- Valid Wisconsin driver's license
- Ability to obtain and maintain security clearance
