Responsibilities
- Provide crisis intervention and case management services during weekend hours
- Conduct home visits and community outreach to vulnerable populations
- Coordinate with multidisciplinary teams to ensure seamless client care
- Maintain accurate documentation and electronic health records
- Participate in weekend on-call rotations for emergency response
- Facilitate weekend support groups and community workshops
- Collaborate with state agencies and non-profit partners
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- Valid Michigan Social Worker License (LMSW)
- Minimum 2 years of direct client service experience
- Experience with crisis intervention and trauma-informed care
- Ability to work independently during weekend shifts
- Strong documentation skills and knowledge of HIPAA compliance
- Valid Michigan driver's license and reliable transportation
