Responsibilities
- Assess client needs and develop individualized service plans for at-risk populations
- Coordinate resources with community partners including healthcare providers and educational institutions
- Maintain accurate case documentation and comply with state/federal reporting requirements
- Provide crisis intervention and emergency assistance services
- Conduct eligibility determinations for public assistance programs
- Participate in multi-agency case reviews and community outreach initiatives
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field
- Minimum 2 years of case management or social services experience
- Kansas driver's license with reliable transportation
- Certification in CPR and First Aid (or willingness to obtain)
- Proficiency with case management software and MS Office Suite
- Excellent communication skills in English and Spanish (preferred)
- Ability to work flexible hours including evenings for client meetings
