Responsibilities
- Client Advocacy: Provide direct support and advocacy to individuals and families, assisting them in navigating state services and community resources.
- Case Management: Maintain accurate and confidential case files, documenting client progress and service delivery in compliance with state regulations.
- Resource Coordination: Connect clients with healthcare providers, housing assistance, and employment opportunities tailored to their needs.
- Community Outreach: Represent the department at community events and workshops to raise awareness about available state programs.
- Crisis Intervention: Assist in de-escalating crisis situations and providing immediate support to clients in distress.
- Administrative Support: Prepare reports, schedule appointments, and manage communication with internal and external stakeholders.
Qualifications
- Education: High School Diploma or GED required; Associate’s degree in Social Work, Psychology, or related field preferred.
- Experience: Minimum of 2 years of experience in social services, case management, or a related field.
- Licensure: Valid California Driver’s License and proof of insurance required.
- Skills: Strong interpersonal communication skills with the ability to build rapport with diverse populations.
- Computer Proficiency: Must be proficient in Microsoft Office Suite and case management software.
- Background Check: Must pass a live scan background check and drug screening.
- Flexibility: Willingness to work flexible hours, including occasional evenings or weekends as needed.
