Responsibilities
- Assess client needs and develop individualized service plans

- Conduct home visits and case management for vulnerable populations

- Provide crisis intervention and emergency response support
Qualifications
- Master's degree in Social Work or related field required
- 2+ years of case management or social services experience
- Kansas Social Work Licensure (LMSW or LCSW)
- Strong crisis intervention and conflict resolution skills
- Proficiency in electronic health records and case management systems
- Valid Kansas driver's license and reliable transportation
- Bilingual (English/Spanish) preferred
