Responsibilities
- Conduct comprehensive initial client intakes and needs assessments to determine eligibility for government programs.
- Develop, implement, and monitor individualized service plans tailored to meet client goals.
- Coordinate with local non-profits, healthcare providers, and housing authorities to secure necessary community resources.
- Maintain accurate and confidential case files, ensuring all documentation complies with federal and state regulations.
- Provide crisis intervention and emotional support to clients facing significant life challenges.
- Advocate for client rights and navigate complex bureaucratic systems on their behalf.
- Attend regular case conferences and training sessions to stay updated on policy changes and best practices.
Qualifications
- Bachelor’s degree in Social Work, Public Administration, Criminal Justice, or a related field is required.
- Minimum of 2 years of experience in case management, social services, or a related government role.
- Valid Kentucky driver’s license and access to a reliable vehicle for site visits.
- Strong knowledge of state and local government resources and funding streams.
- Excellent verbal and written communication skills with the ability to interact effectively with diverse populations.
- Ability to pass a background check, drug screen, and physical examination.
