Job Overview:
As a Government Case Manager, you will serve as the primary point of contact for clients, assessing their needs, developing care plans, and navigating them through complex social service systems. You will work in a dynamic, fast-paced environment that values integrity, compassion, and efficiency.
Responsibilities
- Conduct comprehensive intakes and assessments for new clients to determine eligibility for social service programs.
- Develop and implement individualized case plans tailored to the specific needs and goals of each client.
- Monitor client progress and adjust care plans as necessary to ensure successful outcomes.
- Collaborate with internal agencies, community organizations, and healthcare providers to facilitate client access to resources.
- Maintain accurate and confidential client records in accordance with state and federal regulations.
- Provide crisis intervention and emotional support to clients facing challenging life situations.
- Prepare detailed reports and documentation for case reviews and statistical analysis.
Qualifications
- High School Diploma or GED required; Bachelor's degree in Social Work, Psychology, or a related field is preferred.
- Minimum of 2 years of experience in case management, social services, or a related public sector role.
- Valid driver's license and reliable transportation required for client visits.
- Strong knowledge of Philadelphia's social service landscape and community resources.
- Excellent verbal and written communication skills with the ability to interact professionally with diverse populations.
- Proficiency in Microsoft Office Suite and case management software.
- Ability to work independently as well as part of a multidisciplinary team.
