Responsibilities
- Coordinate client intake and assessment for public health programs
- Develop and implement individualized service plans

Qualifications
- Bachelor's degree in Social Work, Public Health, or related field
- 1+ years case management experience in government/nonprofit
- Valid Maryland driver's license and reliable transportation
- Proficiency with Microsoft Office and case management software
- Certified in CPR and First Aid (or obtain within 30 days)
- Clear background check and drug screen required
- Bilingual Spanish/English preferred
- Ability to work flexible hours including evenings/weekends
