Responsibilities
- Conduct community outreach events to promote senior programs across Manhattan boroughs
- Assist seniors with benefit applications, healthcare navigation, and service enrollment
- Maintain accurate client records using city-mandated databases
- Collaborate with partner organizations for coordinated service delivery
- Develop multilingual educational materials in English and Spanish
- Participate in quarterly inter-departmental task force meetings
Qualifications
- Associate's degree in Social Work, Public Health, or related field
- 1+ years experience in community outreach or social services
- Proficiency in Microsoft Office and case management software
- Valid NYS driver's license with clean record
- Bilingual fluency in English/Spanish required
- Ability to work flexible hours including evenings/weekends
- Clear background check and fingerprinting clearance
