Responsibilities
- Assess client needs and develop comprehensive case management plans
- Provide crisis intervention and counseling services to individuals and families
- Coordinate resources with community organizations and healthcare providers
- Maintain accurate documentation and comply with state/federal regulations
- Advocate for clients within legal and policy frameworks
- Participate in multidisciplinary team meetings and training sessions
Qualifications
- Master's degree in Social Work from an accredited institution
- Kentucky Social Work license required (LCSW preferred)
- Minimum 2 years of direct client service experience
- Strong knowledge of Medicaid and HIPAA compliance
- Excellent communication and crisis management skills
- Ability to work independently and in team environments
- Valid driver's license and reliable transportation
