Responsibilities
- Manage veteran healthcare enrollment and benefits coordination
- Conduct eligibility assessments for VA programs and services
- Provide crisis intervention and psychosocial support to veterans
- Collaborate with interdisciplinary care teams for integrated treatment plans
- Maintain accurate documentation in electronic health records systems
- Coordinate community resources and housing assistance programs
- Facilitate veteran transition services and employment support
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field
- Minimum 2 years' experience in veteran services or healthcare administration
- Proficiency in VA health information systems (VistA, CPRS)
- Valid Wisconsin state driver's license
- Ability to obtain and maintain VA security clearance
- Crisis intervention and trauma-informed care certification preferred
- Strong analytical skills for complex case management
- U.S. citizenship required
