Responsibilities
- Develop and execute neighborhood-based health awareness campaigns
- Coordinate with local organizations to host vaccination clinics and wellness events
- Collect and analyze community health data to inform outreach strategies
- Act as primary liaison between public health initiatives and underserved communities
- Maintain accurate records of outreach activities and resident engagement metrics
- Support grant reporting and program evaluation processes
- Participate in cross-departmental public health initiatives
Qualifications
- Associate's degree in Public Health, Social Work, or related field (or equivalent experience)
- 1+ years community outreach or event coordination experience
- Fluency in English and Spanish (additional languages a plus)
- Strong interpersonal skills with ability to build trust in diverse communities
- Proficiency in Microsoft Office Suite and data collection tools
- Valid Illinois driver's license with reliable transportation
- Ability to work evenings/weekends for community events
- Pass background check and fingerprinting requirements
