Responsibilities
- Develop and deliver engaging health education workshops on topics like disease prevention, maternal health, and nutrition
- Collaborate with community organizations to implement outreach programs in underserved neighborhoods
- Maintain accurate program documentation and participant data using state reporting systems
- Conduct health screenings and provide basic wellness guidance to community members
- Assist in creating culturally appropriate educational materials in multiple languages
- Participate in interagency health initiatives and policy development meetings
- Track program outcomes and prepare monthly impact reports for leadership review
Qualifications
- Bachelor's degree in Public Health, Health Education, or related field (or equivalent experience)
- Minimum 2 years experience in community health outreach or education
- Bilingual proficiency in Spanish or Mandarin strongly preferred
- Certification in Health Education (CHES) or willingness to obtain within 6 months
- Experience with electronic health records and data management systems
- Valid New York State driver's license for occasional field visits
- Ability to work evenings/weekends for community events as needed
- Pass background check and fingerprinting clearance
