Responsibilities
- Assess client needs and develop customized service plans for vulnerable populations
- Coordinate community resources and collaborate with healthcare providers, schools, and nonprofits
- Maintain accurate case documentation and ensure compliance with state/federal regulations
- Provide crisis intervention and trauma-informed support services
- Conduct home visits and community outreach to underserved populations
- Participate in cross-departmental initiatives addressing systemic social challenges
- Track outcomes and prepare reports for program evaluation and funding applications
Qualifications
- Bachelor's degree in Social Work, Public Administration, or related field
- 3+ years experience in social services or public sector role
- Valid Minnesota driver's license and reliable transportation
- Certification in Trauma-Informed Care or Crisis Intervention preferred
- Proficiency in case management software (e.g., Cerner, Epic)
- Knowledge of Minnesota social service programs and policies
- Exceptional communication skills across diverse cultural backgrounds
- Ability to handle sensitive information with confidentiality and discretion
