Responsibilities
- Conduct comprehensive assessments of client needs and develop personalized service plans
- Facilitate crisis intervention and connect families with community resources
- Maintain accurate case documentation and comply with state/federal regulations
- Collaborate with multidisciplinary teams including law enforcement and healthcare providers
- Provide trauma-informed support to vulnerable populations including children and families
- Monitor program outcomes and recommend service improvements
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field
- 2+ years direct case management experience in human services
- Oklahoma Social Work Licensure (LSW/LCSW preferred)
- Certification in crisis intervention and trauma-informed care
- Proficiency with case management software (e.g., CWS/CMS)
- Valid Oklahoma driver's license with clean record
- Bilingual (English/Spanish) candidates strongly encouraged
