Responsibilities
- Provide crisis intervention and case management services during weekend and holiday shifts
- Conduct home visits and assessments for at-risk families across San Francisco
- Collaborate with multidisciplinary teams to develop comprehensive care plans
- Maintain accurate documentation and ensure compliance with state regulations
- Facilitate community resource connections for clients with complex needs
- Respond to emergency hotline calls during assigned weekend shifts
- Participate in weekend outreach programs at community centers
Qualifications
- Master's degree in Social Work or related field from an accredited institution
- Valid California LCSW or LPPC license required
- Minimum 2 years of direct client experience in social services
- Demonstrated ability to work independently during non-standard hours
- Experience with crisis intervention and trauma-informed care
- Proficiency in electronic health record systems (e.g., Cerner, Epic)
- Valid California driver's license with clean record
- Bilingual English/Spanish highly preferred
