Responsibilities
- Provide comprehensive patient care coordination and case management for veteran patients

Qualifications
- Master's degree in Health Administration, Nursing, Social Work, or related field
- Active state licensure (RN, LCSW, or comparable credential)
- Minimum 3 years clinical experience in veteran or underserved populations
- Proficiency with EHR systems (e.g., Cerner, Epic)
- VA experience or federal background preferred
- Valid driver's license and ability to travel between facilities
