Responsibilities
- Coordinate veteran healthcare services and benefits enrollment
- Collaborate with multidisciplinary teams to develop personalized care plans
- Conduct eligibility determinations and benefit counseling sessions
- Maintain accurate electronic health records and documentation
- Advocate for veterans' needs across VA programs and community resources
- Participate in quality improvement initiatives and compliance audits
- Provide crisis intervention and support services to veterans and families
Qualifications
- Bachelor's degree in healthcare administration, social work, or related field
- Minimum 2 years of experience in veterans services or healthcare coordination
- Strong knowledge of VA benefits systems and healthcare regulations
- Excellent communication and interpersonal skills
- Ability to handle sensitive information with confidentiality and empathy
