Responsibilities
- Assist veterans and their families with VA benefit applications, claims processing, and eligibility verification
- Coordinate interdisciplinary care plans and connect veterans with appropriate healthcare services
- Conduct comprehensive needs assessments and develop personalized service plans
- Maintain accurate documentation in electronic health records and benefit tracking systems
- Collaborate with community partners to expand veteran support networks
- Provide crisis intervention and emergency assistance services
- Train veterans and families on available VA resources and digital tools
Qualifications
- Bachelor's degree in Social Work, Public Administration, Healthcare, or related field
- Minimum 2 years of experience in veteran services, healthcare coordination, or social work
- Strong knowledge of VA benefits systems and healthcare regulations
- Excellent communication skills with diverse populations
- Certification in VA benefits navigation preferred
- Ability to work in a fast-paced, mission-driven environment
- U.S. citizenship required for federal position
- Proficiency in electronic health records and case management software
