Responsibilities
- Provide direct patient care and health screenings during weekend shifts
- Administer vaccinations and health assessments at community outreach events
- Maintain accurate electronic health records and documentation
- Coordinate with multidisciplinary teams for seamless patient transitions
- Educate patients on preventive health measures and state programs
- Ensure strict compliance with HIPAA and Illinois health regulations
- Participate in weekend emergency response protocols as needed
Qualifications
- Current Illinois RN or LPN license required
- Minimum 2 years clinical experience in community or public health
- Availability for all weekend shifts (Saturday/Sunday)
- BLS/CPR certification maintained throughout employment
- Proficiency in Epic EMR systems
- Spanish bilingual preferred
- Valid Illinois driver's license for travel to outreach sites
- Ability to lift 25 lbs and stand for extended periods
