The Role:
As the Weekend Case Manager, you will be the first point of contact for families in crisis. You will conduct intakes, perform safety assessments, and connect individuals with vital community resources. This position requires a proactive approach to problem-solving and a deep commitment to public service.
Responsibilities
- Conduct comprehensive intakes and psychosocial assessments for new and existing clients in a timely manner.
- Provide immediate crisis intervention and emotional stabilization to residents facing acute challenges.
- Develop, monitor, and review individualized case plans to ensure client safety and long-term stability.
- Document all client interactions, assessments, and case progress strictly in accordance with government compliance standards.
- Collaborate effectively with law enforcement, hospitals, schools, and other government agencies to coordinate care and resources.
- Facilitate access to housing assistance, food security programs, and employment services for clients.
- Attend weekly team meetings and case supervision to ensure best practices are being followed.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field (Master’s degree preferred).
- Valid Driver’s License and reliable transportation required for client visits.
- Must be available to work Friday, Saturday, and Sunday, including rotating holidays.
- Previous experience working in a government, non-profit, or social services environment is highly desirable.
- Strong interpersonal communication skills with the ability to build rapport quickly.
- Proficiency in Microsoft Office Suite and case management software.
- Ability to handle sensitive information with the utmost confidentiality.
