Responsibilities
- Assess client eligibility for county assistance programs including SNAP, Medicaid, and emergency aid
- Conduct comprehensive intake interviews and maintain accurate case documentation
- Coordinate with community partners to provide wraparound support services
- Process applications within strict compliance guidelines and deadlines
- Develop individualized service plans for vulnerable populations
- Provide crisis intervention and resource navigation for at-risk residents
- Participate in community outreach and public education initiatives
Qualifications
- Bachelor's degree in Social Work, Public Administration, or related field
- Minimum 2 years case management or social services experience
- Proficiency with state eligibility systems (e.g., TWIST, TIES)
- Valid Texas driver's license and reliable transportation
- Strong conflict resolution and crisis intervention skills
- Bilingual (English/Spanish) certification preferred
- Ability to maintain strict confidentiality and HIPAA compliance
- Experience with diverse populations and cultural competency
