Responsibilities
- Client Case Management: Oversee the intake and case planning process for assigned clients, ensuring all documentation is accurate and up-to-date.
- Community Outreach: Conduct home visits and community outreach events to connect individuals with essential social services and resources.
- Administrative Support: Maintain detailed records in the state database, prepare reports for state auditors, and manage correspondence.
- Resource Coordination: Assist clients in navigating the complex bureaucracy of state programs, including healthcare and housing assistance.
- Collaboration: Work closely with other state agencies, non-profits, and local law enforcement to ensure client safety and well-being.
Qualifications
- Education: Bachelor’s degree in Social Work, Public Administration, or a related field preferred; High School Diploma or GED required.
- Experience: Minimum of 2 years of experience in a customer service, administrative, or social services role.
- Skills: Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and familiarity with state government databases.
- Communication: Excellent verbal and written communication skills with the ability to interact effectively with diverse populations.
- Integrity: Strong ethical standards and the ability to handle sensitive and confidential information with discretion.
- License: Valid Pennsylvania Driver’s License and reliable transportation are required for field visits.
