Responsibilities
- Assess client needs and develop comprehensive service plans for vulnerable populations
- Provide crisis intervention and case management for families facing housing instability
- Collaborate with community partners to address systemic barriers to self-sufficiency
- Maintain accurate case documentation and comply with county/state regulations
- Advocate for policy changes that promote equitable resource distribution
- Train junior staff on trauma-informed practices and cultural humility
Qualifications
- Master's degree in Social Work from an accredited institution
- 3+ years of direct service experience in public/social services
- Licensure as a Licensed Social Worker (LSW) in Minnesota
- Demonstrated experience with crisis intervention and case management
- Strong understanding of systemic inequities in housing and healthcare
- Excellent written/verbal communication and documentation skills
- Valid Minnesota driver's license and reliable transportation
