Responsibilities
- Conduct comprehensive client assessments and develop individualized service plans
- Coordinate community resources and referrals for housing, healthcare, and financial assistance
- Monitor case progress and maintain accurate documentation in county systems
- Advocate for clients through multi-agency collaboration and policy navigation
- Provide crisis intervention and trauma-informed support services
- Participate in community outreach and educational initiatives
Qualifications
- Bachelor's degree in Social Work, Psychology, or related field
- 2+ years case management or social services experience
- Oklahoma driver's license and reliable transportation
- Proficiency in case management software (e.g., CWS, MAXIS)
- Certification in CPR and First Aid (or willingness to obtain)
- Strong conflict resolution and cultural competency skills
- Ability to handle confidential information with discretion
