We are currently seeking a dedicated Night Shift Case Manager to join our Human Services team in Oklahoma City. In this vital government role, you will provide essential support and case management services to vulnerable populations during critical evening hours. As a Night Shift Case Manager, you will be the first point of contact for many clients during the evening. Responsibilities
- Conduct comprehensive needs assessments and develop individualized case plans for clients.
- Coordinate care services with healthcare providers, legal aid, and community organizations.
- Maintain accurate, confidential, and timely documentation in the state case management database.
- Respond to crisis calls and emergency interventions during night shifts.
- Facilitate access to housing, food assistance, and employment resources.
- Monitor client progress and adjust case plans as needed to ensure positive outcomes.
- Ensure strict compliance with all federal and state regulations regarding client privacy and welfare.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, Criminal Justice, or a related field (Master’s preferred).
- Valid Oklahoma Driver’s License and reliable transportation.
- Previous experience in case management, social work, or public administration.
- Proficiency with Microsoft Office Suite and electronic case management systems (CRM).
- Strong analytical skills and the ability to make sound decisions under pressure.
- Ability to work independently during night shifts and maintain professional boundaries.
- Clear background check and ability to pass a drug screening.
