Responsibilities

- Coordinate with healthcare providers and social services for integrated care delivery
- Analyze health data trends and prepare actionable reports for county commissioners

Qualifications
- Master's degree in Public Health, Healthcare Administration, or related field
- Minimum 5 years of progressive public health management experience
- Certified in Public Health (CPH) or Healthcare Executive (CHE) designation preferred
- Proven expertise in grant writing and federal program compliance
- Strong analytical skills with health data interpretation experience
- Exceptional leadership and cross-sector collaboration abilities
- Valid Illinois driver's license and ability to travel to community sites
