Key Responsibilities:
- Conduct comprehensive intakes and assessments for new clients to determine eligibility for state assistance programs.
- Develop, monitor, and update individualized case plans that address the specific needs of clients.
- Collaborate with local community agencies, schools, and healthcare providers to coordinate services.
- Document all client interactions and case progress accurately and confidentially in our database systems.
- Provide crisis intervention and advocacy support to ensure client safety and well-being.
- Enforce compliance with state regulations and agency policies.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Criminal Justice, or a related field (or equivalent experience).
- Valid Pennsylvania Driver’s License and reliable transportation.
- Strong written and verbal communication skills.
- Ability to maintain strict confidentiality and handle sensitive information.
- Proficiency in Microsoft Office Suite and case management software.
- Experience working with diverse populations is highly preferred.
