Responsibilities
- Assess client eligibility for state assistance programs including SNAP, TANF, and Medicaid
- Develop and implement individualized service plans for vulnerable populations
- Conduct thorough interviews and investigations to verify case documentation
- Collaborate with community partners to coordinate client support services
- Maintain accurate case records and ensure compliance with state/federal regulations
- Provide crisis intervention and emergency assistance for urgent client needs
- Participate in cross-functional agency initiatives and process improvement projects
Qualifications
- Bachelor's degree in Social Work, Psychology, Public Administration, or related field
- Minimum 2 years of case management or social services experience
- Valid Oklahoma driver's license with reliable transportation
- Proficiency in case management software (e.g., Cerner, MAXIS)
- Strong knowledge of state/federal public assistance programs
- Excellent communication and conflict resolution skills
- Ability to handle sensitive information with confidentiality and professionalism
- Spanish bilingual certification preferred
